Student Forms
Access all official VCAD student forms below. Please read the guidance for each form carefully before submitting.
Complaints Form
This form is for complaints only. A complaint is when you are unhappy with a service, process, or behaviour. You may use this form if:
- You are an applicant or a student.
- You completed Stage 1 (informal resolution) and are dissatisfied with the outcome; or your concern cannot be dealt informally.
If your submission completes successfully, your case will now be reviewed. You will receive an acknowledgement within 10 working days.
Appeals Form
You may use this form if:
- You completed Stage 1 of appeals and are dissatisfied with the outcome.
- It is within 5 working days of receiving your Stage 1 outcome.
If your submission ends unexpectedly, it may be because Stage 1 (Informal Resolution) has not been completed. Please ensure that you have attempted Stage 1 before submitting a Stage 2 formal appeal. If your submission completes successfully, your case will now be reviewed. You will receive a written response within 15 working days.
Extenuating Circumstances Form
Use this form to notify VCAD of any extenuating circumstances that may have affected your academic performance or ability to submit work on time.
Withdrawal Request Form
If you wish to withdraw from your programme of study, please complete this form. Our student support team will be in touch to discuss your decision.
Study Break Request Form
You may request a temporary break from your studies using this form. Please review the study break policy before submitting your request.
Report a Concern
VCAD staff have a collective responsibility to Safeguard our students. Safeguarding applies to anyone under the age of 18 and any adult who could be at risk of abuse or neglect due to care or support needs. Any person could become an adult at risk, so it is important that all staff, including lecturers, are familiar with how their role interacts with students, what they need to be vigilant of, and how to respond to any concern.
Prevent is a statutory duty that aims to safeguard individuals from radicalisation and extremist ideologies and to prevent them from engaging in terrorist activities.
